Three ways to add a note:

  1. When hovering over a comment, press Shift+N
  2. Hover over the author portion of a comment, click the icon
  3. Go to user provile, click 'add note'

Notes will present as tooltips when hovering over the icon by the username in a user comment, and on their profile.
